PCI parallel port card

I installed an I/O Future PP210 two parallel port PCI card and it's not being recognized (DF 2.9.1 master).  I get this in dmesg:

It's based on the NetMOS 9865 chip.  I'm guessing I just need to make an entry for it somewhere and rebuild the kernel (pcidev?).Here's the output of pciconf:
