I just looked at

  http://www.usb.org/developers/devclass_docs/usb_le9.pdf

which, under "Transitioning from Legacy Mode to Extrinsic Support"
refers to "perform[ing] the transition method specified by the host
controller manufacturer".  So it seems to be controller-specific.
