Adam have you tried the drive/cable combo in a different computer?  I had a 
similar problem I was able to trace to a cracked motherboard.  In retrospect, 
it was obvious on close inspection that the motherboard was cracked; however, 
without remove it from the case, it may not be visible, indeed, with todays 
multi-layered PCBs (I think most computer motherboards now are 8 or 16 layer 
fabs) it might not be visible at all.  A cold solder joint on the connector 
can cause the same thing on the motherboard side, as well; I have seen this 
before as well (although not specifically with an ATA-100)
