    libc: Make the signal name array uppercase.
    
    This is to satisfy POSIX which requires that the string printed by
    'kill -l <signal number>' shall be the name of the symbolic constant
    (for example, SIGKILL) without the 'SIG' part.
    
    This also fixes a number of sh(1) tests.
    
    While here, sync a bit further with FreeBSD. Specifically, use
    C99 designated initialization.
