    kernel - Refactor vm_map structure 1/2
    
    * Remove the embedded vm_map_entry 'header' from vm_map.
    
    * Remove the prev and next fields from vm_map_entry.
    
    * Refactor the code to iterate only via the RB tree.  This is not as
      optimal as the prev/next fields were, but we can improve the RB tree
      code later to recover the performance.
