    kernel - xhci cleanup, lock attach
    
    * Lock xhci during attach to avoid spurious interrupts from
      hitting the interrupt routine too early.
    
    * Install interrupt after controller startup, but before
      the main usb code begins querying it.
    
    * Cleanup syntax
