    kernel - Fix swap issue, implement dynamic pmap PT/PD/PDP deletion (4)
    
    * Track down and fix another bug.  pmap_dynamic_delete was imploding
      higher level page table pages still being held by higher call levels.
      Add a pv_hold count check to avoid the situation.
