    kernel - CAM mpsafe issues
    
    * Lock the CAM bus when making asynchronous xpt callbacks.  This code
      previously depended on the MP lock but failed to use the correct
      sim lock, and AHCI/SILI now use their own locks.
    
    * Remove a few other unnecessary mplocks.
    
    * Redo the bus counting code to be MPSAFE.
