    When was the last time you manually fsck'd the filesystems on that
    machine?  It's possible (barely) that prior bugs built up some 
    corruption which the filesystem just now hit.  
It was never manually fsck'd before (the box was installed just a few 
days ago).


    If you could also put the dmesg output for that machine up it would
    be helpful.
