I doubt it; FreeBSD was previously installed on this machine and Hylafax had
no such problem then.

It's not:

# ls -ls /var/spool/hylafax/FIFO.ttyd0

/var/spool/hylafax/FIFO.ttyd0: fifo (named pipe)
